Neck Pain & Cervical Radiculopathy: Clinical Patient Journey

Neck pain is one of the most common musculoskeletal conditions affecting adults and can significantly interfere with work, driving, sleep, exercise, and daily activities. While many cases improve with conservative treatment, pain that persists for several weeks or recurs frequently may indicate an underlying condition that benefits from specialized evaluation.

Neck pain may develop from muscle strain, arthritis, degenerative disc disease, cervical spinal stenosis, poor posture, repetitive movements, or injuries such as whiplash. In some cases, inflammation or compression of nerves in the cervical spine causes pain that radiates into the shoulders, arms, or hands—a condition known as cervical radiculopathy. Depending on the affected nerve, patients may also experience numbness, tingling, or muscle weakness.

Certain symptoms require prompt medical evaluation, including severe weakness, difficulty walking, loss of bowel or bladder control, fever, unexplained weight loss, or neck pain following significant trauma.

2: Diagnosis

Neck pain can originate from muscles, ligaments, spinal discs, facet joints, nerves, or other structures within the cervical spine. A thorough evaluation helps determine the primary source of symptoms and guides treatment decisions.

Comprehensive Physical Examination

Your physician will evaluate:

  • Neck alignment and posture
  • Range of motion
  • Muscle strength
  • Reflexes
  • Sensation
  • Balance and coordination
  • Areas of tenderness
  • Shoulder function
  • Arm strength and grip

Special examination maneuvers may help identify nerve compression, muscle injury, or joint-related pain.

Diagnostic Imaging

Depending on your symptoms, imaging studies may include:

  • X-rays to evaluate arthritis, spinal alignment, or instability
  • MRI to assess discs, nerves, spinal stenosis, and soft tissues
  • CT scans when more detailed evaluation of bone is needed

MRI is particularly helpful when patients have arm pain, numbness, weakness, or symptoms suggesting cervical radiculopathy.

Additional Diagnostic Testing

Some patients may benefit from:

  • Electromyography (EMG)
  • Nerve conduction studies
  • Diagnostic nerve blocks
  • Laboratory testing if inflammatory or infectious conditions are suspected

These tests help distinguish cervical radiculopathy from conditions affecting the shoulder or peripheral nerves.

3: Treatment

Treatment depends on the underlying diagnosis, severity of symptoms, neurological findings, and the degree to which pain affects daily activities.

Most patients improve with nonsurgical treatment.

Activity Modification & Education

Short-term modifications may help reduce irritation while allowing patients to remain active whenever possible.

Recommendations may include:

  • Improving workstation ergonomics
  • Limiting prolonged downward head positioning
  • Adjusting sleep positions and pillows
  • Avoiding repetitive neck strain
  • Gradually returning to normal activities

Physical Therapy & Rehabilitation

Physical therapy is often one of the most effective treatments for chronic neck pain.

Programs may include:

  • Stretching exercises
  • Cervical stabilization
  • Postural retraining
  • Manual therapy
  • Strengthening exercises
  • Range-of-motion exercises
  • Home exercise programs

Improving posture and strengthening supporting muscles often reduces stress on the cervical spine and helps prevent future episodes.

Medications

Medication recommendations may include:

  • Anti-inflammatory medications (NSAIDs)
  • Acetaminophen
  • Muscle relaxants
  • Neuropathic pain medications
  • Topical pain medications

Image-Guided Injections

When conservative therapies do not provide adequate relief, minimally invasive procedures may be recommended.

  • Cervical epidural steroid injections
  • Facet joint injections

These procedures help reduce inflammation, relieve nerve irritation, and identify the source of pain.

Radiofrequency Ablation (RFA)

Patients with chronic neck pain caused by arthritic facet joints may benefit from radiofrequency ablation following successful diagnostic nerve blocks.

This minimally invasive procedure interrupts pain signals from small sensory nerves and may provide relief lasting several months or longer.

Surgery is generally considered only when patients develop progressive neurological deficits, significant spinal cord compression, or persistent symptoms that do not respond to appropriate nonsurgical treatment.

4: Ongoing Management & Prevention

Many patients successfully manage chronic neck pain by combining medical treatment with healthy lifestyle habits that reduce stress on the cervical spine.

Your physician may recommend:

  • Continuing prescribed home exercises
  • Maintaining good posture
  • Regular stretching
  • Ergonomic workstation adjustments
  • Frequent movement breaks during prolonged sitting
  • Strengthening upper back and shoulder muscles
  • Maintaining a healthy weight
  • Stress reduction techniques
