Neuropathic Pain

Neuropathic pain occurs when nerves are injured or irritated and often feels burning, tingling, shooting, or electric. It may be related to diabetes, shingles (post-herpetic neuralgia), nerve compression, surgery, or other neurologic conditions.

At The Medical Group of New Jersey, we identify likely nerve pain sources and use a stepwise plan to improve comfort and function.


Symptom Onset & Initial Evaluation

Common symptoms of neuropathic pain include:

  • Burning, tingling, or “pins and needles.”
  • Shooting pain or hypersensitivity to touch
  • Symptoms are worse at night (in many cases)
  • Numbness or weakness, depending on the cause

Your provider will evaluate sensory changes, strength, reflexes, gait, and potential contributors (blood sugar control, prior injuries, medication effects).


Diagnosis

Neuropathic pain is evaluated through a combination of clinical assessment and targeted testing to identify the source of nerve dysfunction. The diagnostic process typically includes:

  • Detailed clinical history and neurologic examination to assess symptom pattern, sensory changes, and motor involvement
  • Electrodiagnostic testing (EMG and nerve conduction studies) when the diagnosis or exact nerve location is unclear
  • Imaging studies are performed when nerve compression or structural causes are suspected
  • Laboratory testing, in selected patients, to evaluate for contributing or underlying causes

Treatment

Treatment for neuropathic pain is individualized and often requires a multimodal approach focused on symptom control and functional preservation.

Medications (Common First-Line Options)

Medication selection is guided by symptom profile, comorbidities, and tolerance. Common options may include:

  • Nerve-pain medications, such as gabapentin or pregabalin, or SNRIs (e.g., duloxetine), are chosen based on individual needs
  • Topical therapies for localized neuropathic pain in selected cases

Rehabilitation & Interventions

Non-pharmacologic and interventional strategies play an important role in comprehensive care and may include:

  • Physical therapy to maintain strength, balance, and mobility while minimizing pain-related deconditioning
  • Targeted injections or procedures when pain is driven by focal nerve irritation or compression
  • Care coordination with neurology or endocrinology when management of an underlying condition (such as diabetes) is central to symptom control

Ongoing Management & Long-Term Care

Long-term management and prevention of neuropathic pain may include:

  • Regular follow-up to titrate meds and minimize side effects
  • Function-based goals (sleep, walking tolerance, daily activities)
  • Prevention strategies when reversible contributors are identified
