Osteoarthritis & Joint Pain: Clinical Patient Journey

Osteoarthritis is the most common form of arthritis and develops when the protective cartilage within a joint gradually wears away, causing pain, stiffness, swelling, and reduced mobility. Symptoms often worsen over time, making everyday activities such as walking, climbing stairs, exercising, or sleeping more difficult.

At The Medical Group of New Jersey, our Pain Medicine specialists work closely with Primary Care, Rheumatology, Orthopedic Medicine, PM&R, and Physical Therapy to develop personalized treatment plans that relieve pain, improve joint function, and help patients stay active.


1: Symptom Onset & Initial Evaluation

Osteoarthritis usually develops slowly over many years as joints experience normal aging, repetitive use, prior injuries, or excess mechanical stress. While age is one of the strongest risk factors, arthritis can also occur in younger adults following joint injuries or repetitive occupational activities.

Common symptoms include:

  • Joint pain during or after activity
  • Morning stiffness that improves with movement
  • Stiffness after prolonged sitting
  • Swelling around the joint
  • Tenderness when pressure is applied
  • Reduced range of motion
  • Grinding, clicking, or popping sensations during movement

  • Overall medical history
  • How pain affects work, exercise, sleep, and daily activities

This comprehensive evaluation helps determine whether symptoms are caused by osteoarthritis or another condition affecting the joints.


2: Diagnosis

Many conditions can cause joint pain, making an accurate diagnosis essential before treatment begins.

Comprehensive Physical Examination

Your physician will evaluate:

  • Joint alignment
  • Range of motion
  • Joint stability
  • Swelling
  • Areas of tenderness
  • Muscle strength
  • Walking pattern (gait)
  • Balance and flexibility

The examination also helps identify whether pain originates from the joint itself or surrounding muscles, tendons, ligaments, or nerves.

Diagnostic Imaging

Imaging studies may include:

  • X-rays to evaluate joint space narrowing, bone spurs, and arthritis severity
  • MRI when cartilage, ligaments, tendons, or soft tissues require further evaluation
  • CT scans for selected complex cases

Imaging findings are interpreted together with your symptoms and physical examination, as X-ray changes do not always correlate with the amount of pain a patient experiences.

Additional Evaluation

Depending on your symptoms, your physician may recommend:

  • Laboratory testing to rule out inflammatory arthritis or autoimmune conditions
  • Joint aspiration if infection or crystal arthritis (such as gout) is suspected
  • Referral to Rheumatology when inflammatory arthritis is a concern

3: Treatment

Treatment is individualized based on the affected joint, severity of arthritis, overall health, and your functional goals. Most patients benefit from a combination of therapies.

Lifestyle Modifications

Simple lifestyle changes can significantly reduce joint stress and improve long-term function.

Your physician may recommend:

  • Low-impact exercise
  • Activity modification
  • Weight management
  • Supportive footwear
  • Joint protection techniques
  • Assistive devices when appropriate

Even modest weight loss can substantially reduce stress on weight-bearing joints such as the knees and hips.

Physical Therapy & Rehabilitation

Exercise remains one of the most effective treatments for osteoarthritis.

Physical therapy programs may include:

  • Strengthening exercises
  • Flexibility training
  • Balance exercises
  • Range-of-motion exercises
  • Walking programs
  • Functional movement training
  • Home exercise plans

Improving muscle strength around affected joints helps improve stability while reducing pain during daily activities.

Medications

Depending on your condition, medications may include:

  • Acetaminophen
  • Nonsteroidal anti-inflammatory drugs (NSAIDs)
  • Topical anti-inflammatory medications
  • Topical analgesics
  • Selected prescription medications when appropriate

Image-Guided Joint Injections

When symptoms persist despite conservative treatment, minimally invasive procedures may provide additional relief.

Common procedures include:

1: Corticosteroid Injections

Corticosteroid injections help reduce inflammation and may provide temporary pain relief for inflamed joints. These injections are commonly used for:

  • Knee arthritis
  • Shoulder arthritis
  • Hip arthritis
  • Sacroiliac joint pain
  • Other symptomatic joints

2: Viscosupplementation (When Appropriate)

For selected patients with knee osteoarthritis, hyaluronic acid injections may improve joint lubrication and reduce pain. Your physician will determine whether this treatment is appropriate based on current clinical guidelines and your individual condition.

Radiofrequency Ablation (RFA)

For arthritis affecting the spine or selected joints, radiofrequency ablation may provide longer-lasting pain relief.

After successful diagnostic nerve blocks, RFA uses controlled heat to interrupt pain signals traveling through small sensory nerves while preserving muscle function.

Many patients experience pain relief lasting several months or longer following the procedure.

Because osteoarthritis often affects multiple joints, coordinated care may include collaboration with:

  • Rheumatology
  • Orthopedic Medicine

  • Physical Therapy

  • Weight Management specialists

When arthritis becomes severe and significantly limits quality of life despite appropriate conservative treatment, referral for joint replacement surgery may be considered.


4. Ongoing Management & Prevention

Although osteoarthritis cannot be reversed, many patients successfully manage symptoms for years through a combination of medical treatment, exercise, healthy lifestyle habits, and ongoing monitoring.

Your physician may recommend:

  • Continuing regular exercise
  • Maintaining a healthy weight
  • Protecting joints during repetitive activities
  • Strengthening surrounding muscles
  • Remaining physically active
  • Avoiding prolonged inactivity
  • Periodic reassessment of treatment effectiveness

Regular follow-up visits allow treatment plans to evolve as symptoms change and help identify new therapies that may improve function and quality of life.

With early intervention and individualized care, many patients remain active, independent, and able to participate in the activities they enjoy.
