Peripheral Neuropathy: Clinical Patient Journey

Peripheral neuropathy occurs when the nerves outside the brain and spinal cord become damaged, leading to symptoms such as numbness, tingling, burning pain, weakness, or changes in sensation. These symptoms most commonly affect the feet and hands and can interfere with balance, mobility, sleep, and everyday activities.

Diabetes is the leading cause of peripheral neuropathy, although it may also result from chemotherapy, vitamin deficiencies, autoimmune diseases, infections, kidney disease, thyroid disorders, injuries, or nerve compression.

1: Symptom Onset & Initial Evaluation

Peripheral neuropathy often develops gradually, with symptoms beginning in the feet before progressing upward. Some patients notice numbness first, while others experience burning pain, tingling, or increased sensitivity.

Common symptoms include:

  • Burning pain
  • Tingling or “pins and needles” sensations
  • Numbness in the feet, legs, hands, or arms
  • Sharp, stabbing, or electric shock-like pain
  • Increased sensitivity to touch
  • Loss of balance or coordination
  • Muscle weakness
  • Difficulty walking
  • Cramping
  • Reduced ability to sense temperature or injury

Some patients develop symptoms that interfere with everyday activities such as driving, climbing stairs, exercising, sleeping, or maintaining balance.

Because reduced sensation increases the risk of falls and unnoticed injuries, early evaluation is important—even when symptoms are mild.

During your initial consultation, your physician will review:

  • When symptoms began
  • Which areas are affected
  • Whether symptoms are constant or intermittent
  • Medical conditions such as diabetes or thyroid disease
  • Previous injuries or surgeries
  • Current medications
  • Family history of neurologic disorders
  • Alcohol use and nutritional history
  • Prior chemotherapy or cancer treatment

Understanding the progression and pattern of symptoms helps identify potential causes and guides further testing.


2: Diagnosis

Identifying the underlying cause of peripheral neuropathy is one of the most important steps in developing an effective treatment plan. Because many different medical conditions can damage peripheral nerves, a comprehensive evaluation is often necessary.

Comprehensive Neurologic Examination

Your physician will assess:

  • Muscle strength
  • Reflexes
  • Sensation to light touch, vibration, and temperature
  • Balance and coordination
  • Walking pattern (gait)
  • Foot structure and skin integrity
  • Areas of tenderness

The examination helps determine which nerves are involved and whether symptoms are likely caused by peripheral neuropathy or another neurologic condition.

Laboratory Evaluation

Blood tests may help identify medical conditions associated with neuropathy, including:

  • Diabetes and prediabetes
  • Vitamin B12 deficiency
  • Thyroid disorders
  • Kidney disease
  • Liver disease
  • Autoimmune disorders
  • Nutritional deficiencies
  • Certain infections

Treating these underlying conditions may slow or prevent additional nerve damage.

Electrodiagnostic Testing

Your physician may recommend:

  • Electromyography (EMG)
  • Nerve conduction studies (NCS)

These tests evaluate how well nerves transmit electrical signals and help determine the severity and location of nerve damage.

Imaging Studies

Depending on your symptoms, imaging such as MRI or ultrasound may be recommended if nerve compression, spinal disorders, or other structural conditions are suspected.

Some patients with symptoms originating in the spine may undergo imaging to distinguish peripheral neuropathy from lumbar radiculopathy or spinal stenosis.


3: Treatment

Treatment focuses on two important goals:

  • Managing the underlying condition responsible for nerve damage whenever possible
  • Reducing pain and improving daily function

Because nerve damage may not always be reversible, early diagnosis and treatment are important.

Managing the Underlying Cause

Whenever possible, treatment begins by addressing the medical condition contributing to neuropathy.

This may include:

  • Improving blood sugar control in diabetes
  • Correcting vitamin deficiencies
  • Managing thyroid disease
  • Treating autoimmune disorders
  • Adjusting medications when appropriate
  • Addressing alcohol-related nerve damage

Slowing progression is often just as important as relieving symptoms.

Medications for Nerve Pain

Unlike pain caused by inflammation or injury, neuropathic pain often responds best to medications that affect nerve signaling.

Depending on your condition, treatment may include:

  • Neuropathic pain medications
  • Certain antidepressant medications that also treat nerve pain
  • Topical medications
  • Prescription creams or patches
  • Selected pain medications when appropriate

Medication recommendations are individualized based on your symptoms, medical history, and potential side effects.

Physical Therapy & Rehabilitation

Physical therapy can improve:

  • Strength
  • Balance
  • Walking
  • Flexibility
  • Fall prevention
  • Overall mobility

Patients with significant weakness or balance problems may also benefit from assistive devices or gait training.

Interventional Pain Procedures

Although injections are not appropriate for every type of neuropathy, certain patients with localized nerve pain or nerve entrapment syndromes may benefit from:

  • Peripheral nerve blocks
  • Image-guided injections
  • Other targeted interventional procedures

Your physician will determine whether these treatments are appropriate based on the source of your symptoms.

Lifestyle & Supportive Care

Lifestyle modifications often complement medical treatment and may include:

  • Regular exercise
  • Smoking cessation
  • Healthy nutrition
  • Weight management
  • Limiting alcohol consumption
  • Daily foot care, particularly for patients with diabetes

For patients with diabetic neuropathy, careful foot inspections and routine preventive care help reduce the risk of ulcers and infections.


4: Ongoing Management & Prevention

Peripheral neuropathy often requires long-term management. While symptoms may fluctuate over time, consistent treatment can improve comfort, preserve function, and reduce complications.

Your physician may recommend:

  • Regular monitoring of underlying medical conditions
  • Routine medication review
  • Home exercise programs
  • Balance training
  • Fall prevention strategies
  • Protective footwear when appropriate
  • Continued diabetes management
  • Periodic neurologic follow-up

Patients with diabetes should also receive regular foot examinations to identify injuries or pressure areas before serious complications develop.

Many individuals successfully manage neuropathy through a combination of medical treatment, rehabilitation, lifestyle modifications, and coordinated specialty care.
