Peripheral vascular disease (PVD), also known as peripheral artery disease (PAD), occurs when arteries outside the heart, most commonly in the legs, become narrowed or blocked by fatty plaque buildup (atherosclerosis). This reduced blood flow causes pain, fatigue, or cramping in the legs and, if untreated, can lead to ulcers or tissue damage. Fortunately, with early diagnosis and proper treatment, circulation can often be restored, and serious complications prevented.

Symptom Onset & Initial Evaluation

- Leg pain or cramping during walking (intermittent claudication)
- Numbness, weakness, or coldness in the feet or lower legs

Diagnosis & Classification
The goals of diagnostic testing are to confirm PVD, determine the location and severity of blockages, and assess the risk of disease progression.
Vascular Testing
Diagnostic evaluation may include:
- Ankle-Brachial Index (ABI): Compares blood pressure in the ankle and arm to detect blockages. A lower reading indicates reduced blood flow.
- Doppler Ultrasound: Visualizes blood flow through the arteries and helps pinpoint areas of narrowing.
- CT Angiography (CTA) or MR Angiography (MRA): Provides detailed images of blood vessels to identify blockages and plan interventions.
Blood Tests
- Lipid panel (cholesterol profile)
- Hemoglobin A1C (diabetes screening)
- Inflammatory markers that may contribute to vascular disease
Classification of Severity
PVD is classified by symptom pattern and severity:
- Asymptomatic: Artery narrowing is seen on imaging but no symptoms are present.
- Claudication: Pain with exertion that resolves with rest.
- Critical Limb Ischemia (CLI): Persistent pain at rest, ulcers, or tissue loss from severe obstruction.
Because vascular disease often affects multiple arteries, your cardiologist may also screen for carotid artery disease or abdominal aortic aneurysm, which commonly occur alongside PVD.
Treatment & Management
Treatment focuses on restoring healthy blood flow, relieving leg pain, preventing disease progression, and reducing the risk of heart attack and stroke.
Lifestyle & Risk Factor Management
The most important steps in managing PVD include:
- Quit smoking: The single most effective step for improving circulation and preventing progression.
- Adopt a heart-healthy diet: Low in saturated fats and rich in fiber.
- Control blood pressure, cholesterol, and diabetes: Through regular follow-up and medication as needed.
- Exercise regularly: Supervised walking programs or cardiac rehabilitation can significantly improve mobility and circulation.
Medication Therapy
Medications may be prescribed to improve blood flow and slow the progression of atherosclerosis:
- Antiplatelets (aspirin, clopidogrel): Prevent blood clots and improve flow.
- Statins: Slow or reverse atherosclerosis and lower cardiovascular risk.
- Cilostazol or Pentoxifylline: Help reduce leg pain and increase walking distance.
Minimally Invasive & Surgical Procedures
For patients with more advanced disease, procedures may be necessary to restore blood flow:
- Angioplasty with or without stenting: Opens narrowed arteries using a small balloon and may place a stent to keep the artery open.
- Atherectomy: Removes plaque from the artery walls in selected cases.
- Bypass surgery: Creates a new path for blood flow when blockages are severe or extensive.
- Wound care and limb preservation programs: Prevent ulcers and infections in advanced cases.
