Pneumonia is an infection of the lungs that causes inflammation of the air sacs (alveoli), making it more difficult to breathe and exchange oxygen effectively. The infection may be caused by bacteria, viruses, fungi, or aspiration (inhaling food, liquid, or stomach contents into the lungs). Pneumonia can range from a mild illness treated at home to a serious condition requiring hospitalization.

Common types of pneumonia include community-acquired pneumonia (CAP), hospital-acquired pneumonia (HAP), ventilator-associated pneumonia (VAP), and aspiration pneumonia.

Symptom Onset & Initial Evaluation

Pneumonia symptoms can develop suddenly over hours or gradually over several days. The severity of symptoms often depends on the cause of the infection, a patient’s age, and underlying medical conditions.

Common symptoms may include:

  • Cough (dry or producing mucus/phlegm)
  • Fever, chills, or excessive sweating
  • Shortness of breath or rapid breathing
  • Chest pain that worsens with deep breathing or coughing
  • Fatigue or weakness
  • Reduced appetite
  • Wheezing or increased mucus production
  • Confusion or changes in mental status, particularly in older adults

Certain individuals are at higher risk for severe pneumonia, including:

  • Older adults
  • Patients with asthma or COPD
  • Individuals with heart disease
  • Patients with diabetes
  • People with weakened immune systems
  • Smokers

During your evaluation, your provider will review:

  • Current symptoms and their duration
  • Recent respiratory illnesses
  • Travel history and environmental exposures
  • Occupational exposures
  • Smoking history
  • Underlying medical conditions
  • Medications that may affect immune function

A thorough lung examination will be performed to assess for abnormal breath sounds such as crackles, wheezing, or reduced airflow that may suggest infection or inflammation.


Diagnosis & Risk Assessment

Diagnosing pneumonia involves confirming the presence of infection, determining its severity, and identifying the likely cause whenever possible.

Imaging Studies

Chest X-ray (CXR)

  • Often, the first imaging test performed
  • Identifies areas of infection, inflammation, or fluid accumulation within the lungs

Computed Tomography (CT) Scan

  • May be recommended when pneumonia is severe, recurrent, complicated, or not responding to treatment as expected
  • Provides a more detailed evaluation of lung tissue and surrounding structures

Laboratory Testing

Your provider may recommend:

  • Complete blood count (CBC) to assess infection and inflammation
  • Blood chemistry testing
  • Blood cultures in hospitalized or severely ill patients
  • Sputum culture to identify bacterial causes
  • Respiratory PCR testing for viral infections such as influenza, COVID-19, RSV, and other respiratory viruses
  • Pulse oximetry to measure oxygen levels
  • Arterial blood gas testing in selected patients with severe respiratory symptoms

These tests help determine whether the pneumonia is bacterial, viral, fungal, aspiration-related, community-acquired, or healthcare-associated, allowing treatment to be tailored appropriately.


Treatment & Management

Treatment depends on the cause of the infection, symptom severity, oxygen levels, and any underlying health conditions.

Bacterial Pneumonia

When bacterial infection is suspected or confirmed, treatment typically includes:

  • Antibiotics are selected based on the likely organism, patient risk factors, and culture results when available
  • Hydration to help loosen respiratory secretions
  • Medications to manage fever and discomfort
  • Monitoring for symptom improvement and complications

Viral Pneumonia

For viral pneumonia, treatment may include:

  • Antiviral medications for influenza or COVID-19 when appropriate
  • Rest and hydration
  • Symptom management
  • Oxygen therapy if oxygen levels become low

Aspiration, Hospital-Acquired, and Ventilator-Associated Pneumonia

These forms of pneumonia often require more intensive treatment and may involve:

  • Broad-spectrum antibiotics
  • Respiratory therapy
  • Airway clearance techniques
  • Hospitalization for close monitoring
  • Supplemental oxygen or advanced respiratory support when necessary

Supportive Care

Regardless of the cause, supportive treatment may include:

  • Adequate rest
  • Increased fluid intake
  • Cough management when appropriate
  • Breathing treatments or inhalers for patients with airway disease
  • Oxygen therapy when needed

Patients with severe pneumonia, significant oxygen deficiency, advanced age, or multiple medical conditions may require hospitalization for intravenous medications and closer observation.


Monitoring & Follow-Up

Recovery from pneumonia varies from person to person. Some patients improve within a few weeks, while others, particularly older adults or those with chronic lung disease, may require a longer recovery period.

Follow-up care may include:

  • Reassessment of symptoms and breathing status
  • Monitoring oxygen levels
  • Medication adjustments if needed
  • Repeat chest imaging when symptoms persist or risk factors are present
  • Evaluation for underlying lung conditions if pneumonia recurs

The goal of follow-up is to confirm that the infection has resolved and that lung function is returning to normal.


Prevention & Long-Term Lung Health

Preventive care plays an important role in reducing the risk of future pneumonia and related complications.

Recommended prevention strategies include:

  • Staying current on influenza vaccination
  • Receiving recommended pneumococcal vaccines
  • Staying up to date on COVID-19 vaccination when appropriate
  • Practicing good hand hygiene
  • Avoiding smoking and secondhand smoke exposure
  • Managing chronic conditions such as asthma, COPD, diabetes, and heart disease
  • Addressing swallowing difficulties or aspiration risks when present
  • Maintaining good respiratory health through exercise and healthy lifestyle habits

For patients who experience prolonged weakness, shortness of breath, or reduced exercise tolerance after pneumonia, pulmonary rehabilitation may be recommended to help restore strength and lung function.

With prompt diagnosis, appropriate treatment, and ongoing preventive care, most patients recover fully and return to their normal activities.
