Sciatica: Clinical Patient Journey

Sciatica is a common condition caused by irritation or compression of the sciatic nerve, resulting in pain that typically begins in the lower back and radiates through the buttock into one leg. It is most often caused by a herniated disc, spinal stenosis, degenerative disc disease, or bone spurs.

1: Symptom Onset & Initial Evaluation

Sciatica often develops suddenly after lifting a heavy object, twisting awkwardly, or sustaining a back injury. In other cases, symptoms appear gradually as age-related changes in the spine slowly place increasing pressure on the nerves.

Unlike generalized low back pain, sciatica typically affects one side of the body and follows the path of the sciatic nerve.

Common symptoms include:

  • Pain that begins in the lower back or buttock and travels down one leg
  • Sharp, shooting, burning, or electric shock-like pain
  • Numbness or tingling in the leg or foot
  • Muscle weakness
  • Pain that worsens while sitting
  • Increased pain with coughing, sneezing, or certain movements
  • Difficulty standing or walking for prolonged periods

Some patients experience only leg pain with minimal back discomfort, while others develop both lower back pain and radiating leg symptoms.

During your initial consultation, your physician will review:

  • When symptoms began
  • Whether an injury preceded the pain
  • The exact location and pattern of symptoms
  • Activities that worsen or relieve pain
  • Prior episodes of back pain
  • Previous treatments
  • Current medications
  • Occupational and recreational activities

Certain symptoms require immediate medical attention, including loss of bowel or bladder control, rapidly worsening leg weakness, numbness in the groin or saddle area, or severe pain following significant trauma.

2: Diagnosis

Determining the underlying cause of sciatica is essential because treatment varies depending on whether symptoms result from a herniated disc, spinal stenosis, arthritis, or another condition.

Comprehensive Physical Examination

Your physician will perform a detailed examination that may include evaluation of:

  • Walking and balance
  • Muscle strength
  • Reflexes
  • Sensation
  • Flexibility
  • Lumbar spine mobility
  • Hip motion
  • Areas of tenderness

Special examination maneuvers, including the straight leg raise test and other neurologic assessments, help determine whether spinal nerves are being compressed or irritated.

Diagnostic Imaging

Imaging studies may include:

  • X-rays to evaluate spinal alignment and arthritis
  • MRI to identify herniated discs, spinal stenosis, nerve compression, or degenerative changes
  • CT scans when additional detail is needed

MRI is often the preferred imaging study because it provides detailed views of the spinal discs, nerves, and surrounding soft tissues.

Additional Diagnostic Testing

Depending on your symptoms, your physician may recommend:

  • Electromyography (EMG)
  • Nerve conduction studies
  • Diagnostic nerve blocks
  • Laboratory testing when inflammatory or infectious conditions are suspected

These studies help distinguish sciatica from peripheral neuropathy or other neurologic conditions.

3: Treatment

Most patients with sciatica improve without surgery. Treatment focuses on reducing nerve inflammation, relieving pain, restoring movement, and allowing the affected nerve to heal.

Activity Modification & Conservative Care

Although severe pain may require brief periods of rest, prolonged inactivity can delay recovery.

Your physician may recommend:

  • Gradually remaining active
  • Avoiding activities that worsen symptoms
  • Proper lifting techniques
  • Heat or ice therapy
  • Ergonomic adjustments
  • Walking programs

Physical Therapy

Physical therapy is often an important part of treatment.

Programs may include:

  • Stretching exercises
  • Core strengthening
  • Lumbar stabilization
  • Nerve mobilization exercises
  • Flexibility training
  • Postural correction
  • Home exercise programs

Therapy is individualized to reduce pressure on the affected nerve while improving spinal support and flexibility.

Medications

Depending on symptom severity, treatment may include:

  • Nonsteroidal anti-inflammatory drugs (NSAIDs)
  • Acetaminophen
  • Muscle relaxants
  • Neuropathic pain medications
  • Short-term oral corticosteroids in selected cases

Medication recommendations are individualized based on your overall health and medical history.

Epidural Steroid Injections

When symptoms persist despite conservative treatment, epidural steroid injections may reduce inflammation surrounding compressed spinal nerves.

Using fluoroscopic (X-ray) guidance, medication is delivered precisely around the affected nerve root to help relieve pain, improve mobility, and facilitate participation in rehabilitation.

While injections do not correct structural abnormalities, they may provide significant symptom relief for many patients.

Additional Interventional Procedures

Depending on the underlying diagnosis, treatment may also include:

  • Selective nerve root blocks
  • Facet joint injections
  • Sacroiliac joint injections
  • Radiofrequency ablation for associated facet-mediated pain

Your physician will determine which procedures, if any, are appropriate based on your specific condition.

Surgical Consultation

Although most patients recover without surgery, referral to a spine surgeon may be recommended when patients develop:

  • Progressive muscle weakness
  • Significant neurological deficits
  • Persistent disabling pain despite conservative treatment
  • Large herniated discs causing severe nerve compression
  • Symptoms of cauda equina syndrome requiring emergency treatment

When surgery is necessary, it often focuses on relieving pressure on the affected nerve while preserving spinal stability whenever possible.

4: Ongoing Management & Prevention

After symptoms improve, maintaining spinal health can reduce the likelihood of future episodes.

Your physician may recommend:

  • Continuing strengthening exercises
  • Maintaining a healthy weight
  • Regular physical activity
  • Improving flexibility
  • Practicing good posture
  • Proper lifting mechanics
  • Smoking cessation
  • Core conditioning exercises

Follow-up visits allow your physician to monitor recovery, adjust treatment when necessary, and address any recurring symptoms before they become more severe.
