Spinal Cord Injury Rehabilitation

Spinal cord injury (SCI) can cause paralysis or weakness, sensory changes, spasticity, pain, and bowel/bladder challenges. Recovery often requires coordinated, long-term rehabilitation and adaptive planning to maximize independence and quality of life.

Symptom Onset & Initial Evaluation

Following a spinal cord injury (SCI), patients may experience a range of ongoing neurologic and functional challenges that evolve over time. During the initial rehabilitation-focused evaluation, providers assess common post-injury concerns, which may include:

  • Weakness or paralysis, depending on the level and completeness of injury
  • Spasticity and muscle tightness that can interfere with movement and comfort
  • Neuropathic pain, often described as burning, tingling, or shooting sensations
  • Mobility limitations and transfer challenges, affecting independence in daily activities
  • Autonomic concerns in selected patients, along with bowel and bladder management needs

Diagnosis

SCI is initially diagnosed through acute imaging and neurologic examination at the time of injury. In the post-acute and long-term setting, PM&R follow-up focuses on optimizing function and preventing complications through ongoing assessment of:

  • Functional status and adaptive equipment needs, including mobility aids and assistive devices
  • Skin integrity and pressure injury risk, with emphasis on prevention strategies
  • Spasticity, pain management, and seating or wheelchair evaluation to support comfort, safety, and participation

Treatment

Treatment following spinal cord injury focuses on maximizing functional recovery, preventing complications, and supporting independence across the continuum of care. Management is highly individualized and often involves a coordinated, multidisciplinary rehabilitation approach that may include:

  • Physical therapy to address strength, mobility, transfers, balance (when applicable), and endurance
  • Occupational therapy focused on self-care skills, upper extremity function, adaptive techniques, and home or workplace modifications
  • Spasticity management using stretching programs, medications, and targeted interventions when appropriate
  • Neuropathic pain management, incorporating medications and non-pharmacologic strategies to improve comfort and participation
  • Equipment and assistive technology evaluation, including bracing, wheelchairs, seating systems, and mobility aids
  • Bowel and bladder management planning, tailored to neurologic level and individual needs

Ongoing Management & Long-Term Care

Long-term care after spinal cord injury emphasizes maintaining health, preserving function, and adapting to changing needs over time. Ongoing management typically includes:

  • Regular follow-up visits to reassess functional goals, equipment fit, and therapy needs as recovery and life circumstances evolve
  • Prevention of secondary complications, including pressure injuries, contractures, and overuse injuries
  • Education and support for patients and caregivers, focusing on self-management strategies and safety
  • Coordination with primary care and specialty providers to address autonomic issues, chronic pain, and overall health maintenance
