Hypogonadism

Hypogonadism occurs when the body doesn’t produce enough sex hormones—testosterone in men or estrogen in women. This can affect energy, mood, bone strength, fertility, and sexual function. The condition may result from problems in the testes or ovaries (primary hypogonadism) or from hormonal regulation in the brain (secondary or central hypogonadism).

At The Medical Group of New Jersey, our endocrinologists take a comprehensive, individualized approach to identify the cause of hormone imbalance and restore healthy function through targeted treatment and ongoing support.

Symptom Onset & Initial Evaluation

Symptoms vary depending on age, sex, and the underlying cause, but they often develop gradually.

In Men:

  • Fatigue or loss of muscle mass
  • Low libido or erectile dysfunction
  • Mood changes, irritability, or depression
  • Infertility or decreased shaving frequency

In Women:

  • Irregular or absent menstrual periods
  • Hot flashes or night sweats
  • Low libido or vaginal dryness
  • Bone loss or thinning hair

During your first visit, your endocrinologist will review your medical and family history, lifestyle habits, and any prior hormonal treatments. A focused physical exam and lab evaluation will help determine the underlying cause.

Diagnosis

Diagnosis begins with hormone testing, typically performed in the morning when levels are highest. Tests may include:

  • Total and free testosterone (for men) or estradiol, LH, and FSH (for women)
  • Prolactin and thyroid function tests to rule out pituitary disorders
  • Iron studies or MRI of the pituitary gland if central causes are suspected
  • Fertility evaluation when conception is a goal (e.g., semen analysis or ovarian reserve testing)

These results allow your endocrinologist at The Medical Group of New Jersey to determine whether hormone replacement, fertility therapy, or further testing is needed.

Treatment & Ongoing Management

Treatment depends on your diagnosis, age, and personal goals.

Hormone Replacement Therapy

  • Men: Testosterone replacement (injections, gels, or patches) to restore normal levels and alleviate fatigue, low libido, and muscle loss.
  • Women: Estrogen or combined hormone therapy to reduce menopausal symptoms, protect bone health, and improve overall well-being.

Fertility-Focused Treatment

  • Gonadotropin therapy or medications that stimulate natural hormone production for patients planning pregnancy.

Lifestyle & Bone Health

  • Weight-bearing exercise, proper nutrition, and vitamin D supplementation to maintain bone density and muscle mass.

Your endocrinologist will carefully tailor treatment to balance symptom relief, hormone safety, and long-term health.

Follow-Up & Long-Term Care

Regular follow-up visits help monitor hormone levels, adjust dosages, and track treatment response. Periodic blood work ensures that hormone therapy remains effective and safe.

With expert, individualized care from The Medical Group of New Jersey, most patients experience improved energy, mood, and overall quality of life—while protecting long-term bone, cardiovascular, and reproductive health.
