Obesity & Metabolic Syndrome

Obesity is a chronic, progressive condition defined by excessive body fat that increases the risk of serious health problems, including type 2 diabetes, hypertension, heart disease, and sleep apnea. When obesity is combined with high blood pressure, high blood sugar, and abnormal cholesterol levels, the cluster is known as metabolic syndrome.

At The Medical Group of New Jersey, our team provides a compassionate, multidisciplinary approach to weight management, focusing on long-term wellness, prevention of complications, and individualized treatment strategies that help patients achieve and sustain healthy goals.

Symptom Onset & Initial Evaluation

Obesity and metabolic syndrome develop gradually, often over several years. Many patients first notice weight gain, fatigue, or difficulty maintaining energy despite unchanged eating patterns.

During your initial evaluation, your internal medicine care team will:

  • Record height, weight, and body mass index (BMI) to determine obesity classification.
  • Measure waist circumference, a key indicator of metabolic risk.
  • Review dietary habits, physical activity, and sleep quality.
  • Screen for related conditions such as Diabetes Mellitus, Hypertension, and Hyperlipidemia.
  • Order labs to assess glucose, lipid levels, and thyroid function for underlying contributors.

Recognizing obesity as a chronic, treatable medical condition allows your provider to create a comprehensive plan that targets both weight and overall metabolic health.

Diagnosis & Risk Assessment

Diagnosis of obesity is based on body mass index (BMI) and waist circumference, which estimate overall body fat and distribution:

  • Class I: BMI 30–34.9
  • Class II: BMI 35–39.9
  • Class III (Severe): BMI ≥40

When obesity occurs alongside other metabolic abnormalities, such as high blood pressure, high blood sugar, and abnormal cholesterol, it is termed metabolic syndrome, a condition that significantly increases cardiovascular risk.

Your internist at The Medical Group of New Jersey may perform:

  • Fasting glucose and HbA1c: Screens for Diabetes Mellitus
  • Lipid panel: Checks for Hyperlipidemia
  • Blood pressure measurement: Identifies coexisting Hypertension
  • Thyroid testing: Rules out Thyroid Disorders that may contribute to weight changes

These combined findings guide a personalized treatment strategy focused on lifestyle change, medication, and long-term metabolic health.

Treatment & Management

At The Medical Group of New Jersey, our goal is not just weight loss, but improvement in metabolic health and overall well-being. Services provided include:

Lifestyle Modification & Nutrition Counseling

  • Medical nutrition therapy: Customized meal plans emphasizing portion control, nutrient-dense foods, and reduced refined carbohydrates.
  • Exercise planning: Guidance on safe aerobic and resistance exercise programs.
  • Behavioral support: Counseling to address emotional and environmental factors that affect eating habits.

Medication-Assisted Weight Loss

  • If lifestyle changes alone are not sufficient, FDA-approved medications may be prescribed, including GLP-1 receptor agonists (e.g., semaglutide, tirzepatide) for appetite regulation and glucose control.
  • Phentermine-topiramate or bupropion-naltrexone for appetite and craving reduction.

All medication use is closely monitored by your provider for safety and effectiveness.

Advanced & Surgical Options

  • For severe or treatment-resistant obesity, referral to a bariatric specialist may be appropriate. Procedures such as gastric sleeve or bypass surgery can result in substantial, sustained weight loss and improved metabolic health.

Monitoring & Follow-Up

Ongoing support is key to long-term success. Your internist will:

  • Monitor weight, blood pressure, and blood sugar at follow-up visits.
  • Adjust medications as needed for associated conditions like Diabetes Mellitus, Hyperlipidemia, or Hypertension.
  • Encourage participation in The Medical Group of New Jersey’s preventive care and wellness programs for sustainable health maintenance.
