Vascular Trauma

Vascular trauma refers to injury of the arteries or veins caused by accidents, penetrating wounds, fractures, or medical procedures. These injuries can lead to bleeding, poor circulation, or tissue loss, requiring urgent diagnosis and repair to prevent limb or life-threatening complications.

At The Medical Group of New Jersey, our vascular surgeons are on call for emergency vascular trauma care, providing rapid surgical and endovascular repair of arterial and venous injuries in coordination with trauma and orthopedic teams.


Symptom Onset & Initial Evaluation

Vascular trauma refers to any injury that damages an artery or vein, leading to bleeding, restricted blood flow, or tissue damage. Symptoms vary depending on the location and severity of the injury, but may include:

  • Uncontrolled bleeding or an expanding hematoma at the injury site
  • A cool, pale, or pulseless limb, indicating loss of circulation
  • Severe pain, tingling, or numbness in the affected area
  • A whooshing sound (bruit) or vibration (thrill) over the wound, which may suggest an abnormal artery-vein connection (AV fistula)
  • Swelling or tightness in the limb due to rising compartment pressure

At the time of evaluation, the medical team focuses on immediate stabilization and rapid assessment of blood flow to the injured area. Diagnosis

Accurate diagnosis is critical to determining the extent of vascular injury and planning the appropriate repair. Evaluation typically combines a targeted physical exam with advanced imaging.

  • CT Angiography (CTA): The fastest and most precise method to identify arterial injury, blockage, or pseudoaneurysm formation.
  • Duplex Ultrasound: Used in stable patients to assess superficial arteries and veins for injury or thrombosis.
  • Arteriography: Performed during surgery to provide real-time visualization and guide repair techniques.

Treatment

The goals of vascular trauma treatment are to control bleeding, restore circulation, and preserve limb and organ function.

Emergency & Surgical Repair

When immediate repair is required, vascular surgeons may perform one or more of the following procedures:

  • Primary repair or end-to-end anastomosis: Directly reconnects damaged vessel ends to restore blood flow.
  • Interposition grafting: Uses a vein or synthetic graft to bridge gaps in large or complex vessel injuries.
  • Bypass surgery: Creates an alternate pathway for blood flow when direct repair is not possible.
  • Fasciotomy: Relieves pressure buildup within muscle compartments to prevent ischemic tissue damage.

Endovascular & Minimally Invasive Techniques

For select patients, minimally invasive options can restore circulation or control bleeding without open surgery:

  • Stent graft placement: Reinforces injured arteries or seals pseudoaneurysms using a catheter-based approach.
  • Embolization: Blocks bleeding vessels internally using targeted coils or plugs.
