Symptom Onset & Initial Evaluation

During your evaluation, the neurologist will:

  • Review your headache history, triggers, and response to prior medications
  • Perform a neurological exam to rule out other causes
  • Order imaging (MRI or CT scan) if there are “red-flag” warning signs such as weakness, confusion, or new-onset headaches after age 50

If you experience a sudden, severe headache (“the worst headache of your life”), new neurological symptoms, or a headache that follows a head injury, seek emergency care immediately.


Diagnosis & Subtype Classification

Most chronic headaches are primary, meaning they are not caused by another disease. Common subtypes include:

  • Migraine: Recurrent, often one-sided, throbbing pain with nausea and sensory sensitivity.
  • Tension-type headache: Steady, pressure-like pain on both sides of the head, often linked to stress or muscle tension.
  • Cluster headache: Severe, stabbing pain around one eye, often occurring in cycles or “clusters.”

Diagnosis is based on your symptom pattern and medical history. Your neurologist may recommend keeping a headache diary to identify triggers and track progress.


Treatment & Management Plan

Treatment focuses on both stopping attacks and reducing their frequency.

Acute (Abortive) Therapies:

  • Over-the-counter pain relievers or prescription triptans for migraines
  • Gepants and ditans, newer migraine-specific options for patients who cannot take triptans
  • Oxygen therapy or injectable triptans for cluster headaches

Preventive (Prophylactic) Therapies:

  • Daily medications such as beta-blockers, antiepileptic drugs, or antidepressants
  • Monthly or quarterly CGRP inhibitors (Aimovig®, Emgality®, Vyepti®) to prevent migraine onset
  • Botox® injections for chronic migraine (15+ headache days per month)

Lifestyle & Trigger Management:

  • Maintain regular sleep, hydration, and meal schedules
  • Limit caffeine and alcohol, and avoid skipped meals
  • Practice stress-reduction techniques such as yoga, mindfulness, or biofeedback

Advanced & Integrative Care Options

For patients with refractory headaches, The Medical Group of New Jersey offers:

  • Infusion therapy for prolonged migraine attacks (status migrainosus)
  • Neuromodulation devices, such as noninvasive vagus-nerve or trigeminal stimulators
  • Nutritional and hormone assessments for patients with migraine related to diet or menstrual cycles

Your neurologist may also collaborate with a pain specialist or physical therapist to address muscle tension, posture, or cervical spine involvement that can contribute to headaches.
