Symptom Onset & Initial Evaluation
Myasthenia gravis (MG) is an autoimmune disorder that affects communication between nerves and muscles, causing weakness that worsens with activity and improves with rest.
Common early signs include:
- Drooping eyelids (ptosis) or double vision (diplopia)
- Difficulty chewing, swallowing, or speaking clearly
- Arm or leg weakness that increases with use
- Shortness of breath, especially after exertion
If you experience sudden breathing difficulty, seek emergency care immediately; this may indicate a myasthenic crisis, a life-threatening condition requiring urgent treatment.
During your first visit, your neurologist will:
- Perform a detailed neurological and strength examination
- Order antibody tests for acetylcholine receptor (AChR) and MuSK antibodies
- Conduct repetitive nerve stimulation or single-fiber EMG to confirm neuromuscular junction dysfunction
- Recommend a chest CT or MRI to evaluate the thymus gland for thymoma (a tumor often associated with MG)
Diagnosis & Subtype Classification
Myasthenia gravis can be classified based on antibody type and distribution of symptoms:
- AChR-positive MG: Most common form; responds well to standard therapies.
- MuSK-positive MG: Often affects facial, neck, and respiratory muscles; may need different medications.
- Seronegative MG: No detectable antibodies, but diagnosis confirmed by EMG findings.
- Ocular MG: Weakness limited to eye muscles, sometimes progressing to generalized MG.
Your neurologist will combine test results and symptom patterns to confirm the diagnosis and tailor a treatment plan.
Treatment & Ongoing Management
With modern therapies, most patients achieve significant improvement or remission.
Symptomatic Therapy:
- Pyridostigmine (Mestinon®): Improves nerve-to-muscle communication and relieves fatigue.
Immunosuppressive Therapy:
- Corticosteroids (prednisone) to reduce immune activity.
- Steroid-sparing agents such as azathioprine, mycophenolate mofetil, or cyclosporine for long-term control.
- Monoclonal antibody treatments (eculizumab, ravulizumab, or rituximab) for refractory cases.
Rapid-Acting Treatments (for crisis or severe symptoms):
- Intravenous immunoglobulin (IVIG) or plasmapheresis to quickly remove circulating antibodies.
Surgical Management:
- Thymectomy (removal of the thymus gland): Recommended for many patients, even without a thymoma, to improve long-term outcomes.
Rehabilitation & Supportive Care
Living with MG requires a careful balance between activity and rest.
- Physical therapy: Helps maintain endurance and prevent deconditioning.
- Speech and swallowing therapy: Assists patients with bulbar symptoms.
- Respiratory monitoring: Regular pulmonary function testing ensures early detection of respiratory decline.
- Lifestyle management:
- Get adequate rest and avoid overheating.
- Inform all healthcare providers about your diagnosis—some medications can worsen MG (e.g., certain antibiotics or beta-blockers).
- Stay up to date on vaccinations to reduce infection-related flare-ups.
Prognosis & Long-Term Monitoring
With ongoing care, most people with MG lead normal, active lives. Regular follow-ups allow your neurologist to monitor response to therapy, adjust medications, and prevent complications.
